Understanding the Incident


On March 24, 2023, tragedy struck West Reading, Pennsylvania, when an explosion ripped through an R.M. Palmer Company candy factory. The source, a natural gas leak from a natural gas pipeline. The blast tragically claimed seven lives, injured ten, and caused severe damage to nearby structures, displacing families in the process. This accident highlights a series of preventable oversights and underscores the importance of proactive safety measures.


Key Findings from the NTSB


  1. Pipeline Integrity Issues: The retired Aldyl A polyethylene service tee, left connected to the gas distribution system, became the point of gas leakage due to material degradation exacerbated by elevated temperatures from a nearby corroded steam pipe.

  2. Elevated Temperature Risks: Operators lacked awareness of threats posed by high-temperature environments. Existing integrity management programs failed to address these hazards effectively.

  3. Emergency Preparedness Gaps: R.M. Palmer’s employees were unprepared for a gas leak emergency. Many remained in the building after detecting the smell of gas due to insufficient training and lack of clear evacuation procedures.

  4. Public Awareness Deficiencies: Safety guidelines often failed to reach all affected individuals, emphasizing the need for targeted, inclusive communication strategies.


Recommendations for Change


In response to its findings, the NTSB has issued a range of recommendations aimed at improving safety, awareness, and preparedness:

  • Pipeline Safety: The Pipeline and Hazardous Materials Safety Administration (PHMSA) is urged to mandate inventories and risk assessments for plastic assets in elevated temperature environments, encouraging preventive measures.

  • Enhanced Public Communication: Improvements in public safety programs to reach all members of the community and guidelines for installing natural gas alarms are critical.

  • Emergency Preparedness: Employers using natural gas must implement clear procedures and training for emergency scenarios. Revising state laws and codes to require natural gas alarms and improve underground pipeline awareness is essential.

  • Regulatory Updates: Updates to industry standards and more robust maintenance practices, including critical valve assessments, are vital to prevent future tragedies.
